Time Off Request Form

We know that there are days you will have conflicts in your schedule and that you won’t be able to teach your students. The expectation is that you will give GPA a two weeks or greater notice so that we have time to find a substitute teacher / schedule your students for a makeup lesson day.

Please fill out this form requesting your dates off. You can request more than one date. If you know all of your dates for an entire semester, that helps us! The earlier you notify us, the better!

If we are not able to find a substitute for you, we will need you to offer a Friday or Saturday as a makeup day. Please list days you can offer as makeup lessons. These dates need to be as close to your absence as possible, either before the absence or directly after the absence.
